Q1: Beginning on Jan. 1, 2004 by showing the NHI IC Card when visiting a doctor
  • Beginning on Jan 1, 2004, the insured are encouraged to NHI IC cards when visiting doctors. If you have not had your IC Card yet, please call the NHI IC Card Service Line at 0800-030-598 for assistance.
  • For newborns or first-time enrollees, please fill out the “IC Card Application Form” with your NHI registration (remember to attach a copy of your ID card on the back of the application form, and your photo if possible). Mail or bring it in person to a nearby NHI branch office or liaison office. Or, call the NHI IC Card Service Line (0800-030598) for further information.
  • In addition, since the NHI IC Card consolidates various health insurance cards, after Jan. 1, 2004, the children's health handbook and the maternity health handbook will be issued by the Bureau of National Health Promotion instead.

Q2:What to do if you don't have a NHI IC Card?
  • The paper-made NHI Card is no longer accepted at hospitals and clinics, since Jan. 1,2004, NHI IC Card is the only valid healthcare card thereafter.
  • If you have already enrolled in the NHI program, but have not had your NHI IC Card yet, or you are in applying a new card due to name changes, card deteriorations, photo changes, lost, and etc. What should you do when you go to see a doctor? You only have to fill out the “Exceptional Medical Seeking Name List” at the clinic or the hospital. Thus your benefits coverage will remain the same as the insured.
  • Please fill out the "NHI IC card Application Form" as soon as possible ( forms are available in all hospitals, NHI branch offices, liaison offices, and post offices ). Please attach your photo and a copy of your ID with the application form either mail or bring it in person to a nearby NHI branch office. The Bureau will issue your NHI IC Card in prompt. If you have any questions, please call the NHI IC Card Service Line at 0800-030-598.

Q3:What are the specifications of the photo for the NHI IC Card? Can I use a photo that has different specifications?
  This photo should be 2-inch, colored/black-and-white and taken within 2 months. On the photo, the figure shall be without a hat and shall not wear the colored glasses. We don't accept any photo other than a certificate photo. If the specifications of your photo are almost close to the above-mentioned but only the size is 1 inch which is acceptable. However, its resolution on the card will not as good as that of the 2-inch's.

Q4:What are contents of the NHI IC Card?

  There are four sections of information stored in NHI IC Card. They are section of 1)personal information; 2) NHI-related information;3) medical services, and 4) public health administration. Ccontents are as follows:

  • The Personal Information Section mainly includes the cardholder's name, gender, date of birth, ID number and picture.
  • The NHI-related Information Section mainly includes remarks of the cardholder's status and/or catastrophic diseases, number of visits and admissions, utilization of the NHI health prevention programs, premium records, accumulated medical expenditures, and the amounts of cost sharings.
  • Medical Services Section mainly includes drug allergy history and long-term prescriptions of ambulatory care and/or certain medical treatments. Information of this section will recorded gradually in regarding of how the healthcare providers adapt themselves to this system.
  • Public Health Administration Section mainly includes personal immunization records and willingness of organ donation.

Q5:How to apply another NHI IC Card if the original one had lost or damaged? Should I pay any handling fees?
  Please contact your group insured unit directly for helping you to apply a new card. Or you may visit our Service Center at our Branches for applying a new card.

Q6 :When should I start using the NHI IC Card?
  You are encouraged to bring your NHI IC card with you when visiting a doctor since July 1,2003. A photo ID will be needed to identify yourself if you hold a non-photo NHI IC Card.

Q7:How do I renew my NHI IC Card If I have used it for six times already?
  You may renew your card at a Public IC Card Kiosks located at BNHI's branch offices or affiliated outpatient centers, hospitals, or local town houses.

Q8:What to do if I do not have my NHI IC Card?
   Same as Q5.

Q9:What should I do if my card's validity is expired”?

Your NHI IC Card should be renewed once a year. Its validity period ends on your birthday of the next year. You may renew the card one month prior to your birthday on next year by accessing any NHI IC card readers. Hospital staffs are pleased to help to renew it.

Q10:Is it necessary to bring my NHI IC Card with me when seeking Chinese injury care, acupuncture treatment, or dental root canal?
  Yes, the NHI IC Card will be swiped even for treatments of the same episode of diagnosis, however this visit will not be counted.

Q11:Can I set a password for the NHI IC Card? And how do I set my personal emergency phone number?
  Since there is no medical history built inside the IC card, we do not suggest you to set a password for now. However, you may set password and/or personal emergency phone number by using card readers either located on BNHI Branches or local town houses.

Q12:Is there any possibility that my medical information will be revealed without permission when using the NHI IC Card?
  The NHI IC Card can only be read by the NHI IC Card reader when accompained by a verified SAM card. Data stored in the cards cannot be read without going through a strict authorization and mutual recognition security process. Besides, there will not be any medical history recorded in the NHI IC Card so far.

  Same as Q5.
Q14:Do I need to apply for a new card if I changed my group insured unit?
  No, you do not have to.
